- Deciding Where to Start
For an aging parent in Nashville, the first decision is where care happens: at home, with an agency sending caregivers for a few hours or around the clock, or in a senior living community. Five Nashville senior living communities and five home care agencies that serve the city split along that line. Before choosing, you can check a facility’s state license and inspection record, and Tennessee’s TennCare CHOICES program can pay for long-term care at home or in a nursing facility for people who qualify.
Locations and services for each community and home care agency are given as the provider describes them on its website in September 2026.
Care at Home or in a Community
The two paths answer different needs.
Home care agencies send caregivers to the house. The agencies describe a range of schedules: Comfort Keepers offers care “from a few hours a week to 24/7 care,” and Visiting Angels lists “hourly care, overnight care and 24-hour care.” Their services center on daily living. Visiting Angels’ list includes “Respite care Companionship Personal care Hygiene assistance Meal planning and preparation Light housekeeping Laundry assistance Medication reminders Assistance running errands.” Touching Hearts at Home describes its work as “non-medical care.” If a parent needs skilled nursing or rehabilitation, ask any agency what it can and cannot provide at home.
Senior living communities provide care where the resident lives. Blakeford, Richland Place, The Meadows, and Belmont Village each list more than one level of care on the same campus. Ask any community how a move between levels works if a parent’s needs change.
What Each Community Offers
| Community | Levels of care it lists |
|---|---|
| Blakeford Senior Life | Independent living, assisted living, memory care, skilled nursing and rehab, and in-home care through Blakeford at Home |
| Richland Place | Apartments plus a state-licensed Health Center with beds for "skilled nursing, rehab, intermediate care, and inpatient hospice services" |
| The Meadows Senior Community (NHC) | Short-term skilled nursing and rehabilitation, assisted living, memory care, independent living |
| Belmont Village Green Hills | Assisted living, early memory care, advanced memory care, with licensed nurses on site 24/7 |
| Mary, Queen of Angels | Faith-based, non-profit assisted living |
After a Hospital Stay
Blakeford, Richland Place, and The Meadows list skilled nursing and rehabilitation for recovery after a hospital stay. The Meadows says many of its residents “are short-term, having left a hospital needing skilled nursing and/or rehabilitative care to get them back home.” Ask the hospital’s discharge planner whether a bed is available and how the stay would be paid.
Memory Care
Blakeford, Belmont Village Green Hills, and The Meadows name memory care. Blakeford offers “assisted living and specialized memory care” at Burton Court at Blakeford. Belmont Village Green Hills separates “Early Memory Care” from “Advanced Memory Care.” The Meadows lists memory care on its campus. Among home care agencies, Cornerstone Caregiving lists “Alzheimer’s & Dementia Care,” and Comfort Keepers says it offers Alzheimer’s and dementia care at home.
Check a Facility’s License and Inspection Record
In Tennessee, the Health Facilities Commission’s Division of Licensure and Regulation “licenses and/or certifies 21 types of health care facilities.” For facilities that receive Medicare or TennCare, the commission says “a link to CMS Statement of Deficiencies or facility survey results” is available on its Public Record and Complaints page. Its number for licensure questions is 615-741-7221.
Before a tour, look up the community’s most recent survey. Bring any deficiency you find to the tour and ask what changed.
Paying Through TennCare CHOICES
TennCare’s CHOICES program “provides older adults (age 65 and older) & adults with physical disabilities (age 21 and older) who are eligible for needed long-term services and supports in the home/community setting or in a nursing facility.” TennCare sorts members into three groups:
- Group 1: people of all ages who receive nursing home care.
- Group 2: seniors and adults with a physical disability who qualify for nursing home care “but choose to receive home care services instead.”
- Group 3: seniors and adults with a disability who don’t qualify for nursing home care “but still need care to delay or prevent the need for nursing home care.”
TennCare publishes its financial limits for Medicaid long-term services and supports. For 2026, it says income “can’t be more than $2,982 per month” (a figure that “changes annually”), the total value of things you own “can’t be more than $2,000 (The home where you live doesn’t count),” and “You can’t have given away or sold anything for less than what it’s worth in the last five (5) years.” Check the TennCare page for the current year’s figures before you apply.
To apply, TennCare says to call 1-866-836-6678 “from anywhere in the state to be automatically directed to your nearest Area Agency.” In the Nashville area, that agency is the Greater Nashville Regional Council’s Area Agency on Aging and Disability, whose Information and Assistance Helpline is also at 615-255-1010.

Deciding Where to Start
Start with the need: help at home for part of the day, round-the-clock care, memory care, or recovery after a hospital stay. Call the Area Agency on Aging and Disability if TennCare may pay, check the survey record of any community you tour, and ask each provider the same five questions.
